What Happens If You Missed Open Enrollment? (2026)

Short answer: nothing punitive — there's no federal penalty for being uninsured, and Illinois doesn't have a state mandate either. But "no penalty" isn't the same as "no problem." One ER visit without coverage averages thousands of dollars, and a hospital stay can reach tens of thousands. So the real question isn't what happens to you — it's how to get covered anyway. There are four ways.

The situation

Open enrollment for 2026 coverage in Illinois ran through January 31, 2026 on the state's new Get Covered Illinois marketplace. If you didn't enroll, the marketplace's front door is closed until fall 2026 (for coverage starting January 2027) — unless a side door applies to you. Most people qualify for more than they think.

Door #1: A qualifying life event (most common)

Life changes reopen enrollment for 60 days. If any of these happened recently — or happens later this year — you can enroll in a full ACA plan on the spot:

  • Lost job-based coverage, aged off a parent's plan at 26, or lost Medicaid
  • Got married
  • Had or adopted a baby
  • Moved to a different coverage area
  • Became pregnant (Illinois now treats pregnancy itself as a qualifying event)

Two things people get wrong: the 60-day window runs from the event, not from when you find out about the rule; and losing coverage because you stopped paying your premium generally does not count.

Door #2: Medicaid — no deadline exists

Medicaid enrolls 365 days a year. Eligibility is by household income. If money is tight this year, check this first — it costs nothing to find out and nothing to have.

Door #3: Year-round private health plans

If no life event applies and your income is above Medicaid range, licensed agents offer private health plans you can start now — often within days.

Plain-spoken trade-off: these aren't ACA plans. They may involve health questions, and pre-existing conditions may not be covered. They fit best as a bridge to the next open enrollment, or for healthy people priced out of full-cost ACA premiums. (And know your rights: short-term plans can't legally be sold in Illinois — anything legitimate an Illinois agent offers will be a different product type.)

Door #4: Wait for fall — but set a reminder now

Next open enrollment starts November 1, 2026. If you choose to wait, do two things: put the date in your phone today (deadlines have been moving targets the past two years), and consider at least accident/critical-illness supplemental coverage in the meantime so a broken arm doesn't become a broken bank account. Supplemental plans aren't health insurance, but they're cheap and they enroll year-round.
